The facts set forth in my application for employment are true and complete. I understand that if any of the information in this application is false or misleading, Chadwick-BaRoss Inc. may deny or terminate employment. I understand in Massachusetts, it is unlawful to require or administer a lie detector test as a condition of employment or continued employment, and that an employer who violates this law shall be subject to criminal penalties and civil liability. I understand this application is not intended to be a contract of employment. I understand that any offer of employment is contingent on satisfactory results from a Driver’s Record check, Physical and Substance Abuse Test, Reference Check and Behavioral and Aptitude Assessments. I understand this application is not intended to be a contract of employment. I understand under federal law, employment will be terminated for failure to present documentation establishing my identity and eligibility for employment in the U.S. I understand that if employed I will be an employee-at-will, and my employment can be terminated by me or Chadwick-BaRoss Inc. with or without cause at any time. I further authorize ChadwickBaRoss Inc. to contact any party listed above to release any information concerning prior work experience and I release them and their representatives from any and all liability for providing such information. If there is a particular employer you do not wish us to contact, please indicate which one.

DISCLOSURE TO APPLICANTS AND EMPLOYEES REGARDING PRE & POST EMPLOYMENT SCREENING, BACKGROUND CHECKS AND OTHER REFERENCE CHECKS OR REPORTINGS
In processing your application for employment and otherwise in connection with making certain employment decisions, including promotion, reassignment, or whether to continue your employment (if you are hired), Chadwick-BaRoss Inc. may from time to time procure or have prepared a report of your motor vehicle record history.
By your signature below, you acknowledge this disclosure that Chadwick-BaRoss Inc. may obtain your motor vehicle record history for employment purposes. You further authorize Chadwick-BaRoss Inc. to contact any party listed on your job application and/or professional references obtained by you to release any information concerning prior work experiences and you release them and their representatives from any and all liability for providing such information. You hereby authorize Chadwick-BaRoss Inc. to obtain such reports and cause such investigation into these matters as it deems necessary for employment purposes. You authorize all persons, entities, and agencies to release information to Chadwick-BaRoss Inc. or its agents for the purposes stated herein. You hereby authorize Chadwick-BaRoss to utilize behavioral analysis assessments in determining your anticipated success in employment with Chadwick- BaRoss, Inc. This Acknowledgement and Authorization is intended to cover any pre- employment screenings, post-employment screening, background checks, and other reference checks or reports. This includes any that may be necessary or conducted during your employment, if you are employed.
Applicants being considered for hire must pass a drug and physical abilities test before beginning work with Chadwick-BaRoss, Inc.
Chadwick-BaRoss Inc. requires completion of this Acknowledgement and Authorization as a condition of employment or consideration for employment.
